Japanese Governments and other developed countries try to control payment to health care providers facing expansion of medical expenditure. Although the diagonis-related groups have attracted Japanese health authorities and have been introduced in university hospitals and national centers of excellence, the majority of hospitals and clinics are paid by fee for service which is called the "Point-fee System". The Ministry of Health, Labor and Welfare sets the fee schedules that serves as the unique table for the remuneration of the medical facilities. It serves as the control bulb for the global budgeting as well as to give incentives to physicians for the desirable services. This article discusses the history and the present state of the Japanese payment scheme. (résumé d'auteur).
