A number of the articles in this volume are not academic in their approach : they are "state of the art papers", written by practitioners who are working at the sharp end of developing accreditation systems... They demonstrate how vitally important it is to have clear views of what accreditation is, and how the accrediting organisations have to concentrate upon the details of their own internal structures and processes if they are to succeed in their objectives. These papers therefore mark a turning point in accreditation. They do not discuss the system of accreditation which has been accepted for the past forty years - instead they address the present and the future. It is possible that they mark the end of accreditation as they all document a rejection of the standard, typically associated with accreditation... The development of clinical performance indicators is possibly a step on the road to the introduction of outcome measures into accreditation. If this turns out to be the case, accreditation system will have moved away from assessing the suitability of the health service to achieve care to a monitoring system which assesses organisational success in terms of outcome measures... Is this change a natural progression in the evolution of accreditation systems or does it reflect that accreditation is an outmoded concept of how quality should be assured ?
